INFORMATION CLAUSE - VIDEO SURVEILLANCE

Pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) ("RODO"), we kindly inform you that:

  1. The administrator of your personal data is Klinika Phlebologii spółka z ograniczoną odpowiedzialnością with its registered office in Warsaw, ul. Wawelska 5, 02-034 Warsaw, holding tax identification number (NIP): 7010498962, entered in the register of entrepreneurs kept by the District Court for the City of Warsaw in Warsaw, XII Economic Division of the National Court Register under KRS number: 0000568625 ("Administrator").   1. The Controller has appointed a Data Protection Officer. The Data Protection Officer is responsible for all matters relating to the processing of personal data by the Controller. If you have questions about how or to what extent your personal data is being processed in the course of the Administrator's activities, or about your rights in relation to it, you may contact the Data Protection Officer:

The Administrator processes your personal data in the form of your image and the time and place of the incident in order to increase the safety of patients, the Administrator's employees and other persons in the monitored area, as well as to increase the protection of the property located there, on the basis of Art. 6(1)(e) RODO (processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the Administrator) and Art. 6(1)(f) RODO (processing is necessary for the purposes of the legitimate interests pursued by the controller or by a third party, except where such interests are overridden by the interests or fundamental rights and freedoms of the data subject̨ which require protection of personal data);
  2. Video monitoring shall include entrances to and exits from the Administrator's treatment facility, circulation routes, and the registration and waiting area. Signage indicating that the facility is being monitored is located within the Administrator's treatment facility;
  3. Personal data is recorded by surveillance cameras, which record the image continuously. Only the image is recorded and stored. Access monitoring records the entries and exits of authorised persons;
  4. personal monitoring data are stored for a period of 3 months;
  5. The Administrator secures and makes personal data available only to the relevant authorities authorised under the relevant legislation, e.g. the police, the prosecutor's office or the court. Apart from these entities, the recipients of your personal data may be employees and associates of the Administrator. The Administrator does not make your personal data available to other recipients;
  6. The Administrator shall keep your personal data confidential and protect them from unauthorised access by third parties in accordance with the principles set out in the applicable legislation;
  7. Your personal data will not be transferred to a third country (outside the European Economic Area) or to an international organisation;
  8. Your personal data will not be processed by automated means and will not be profiled;
  9. In accordance with the principles set out in the personal data protection legislation and to the extent provided therein, the data subject has the right to: access personal data concerning him/her, rectify (amend) them, complete them if they are incomplete, erase or restrict the processing, transfer and receive a copy, object to the processing or withdraw consent to the processing of personal data (if the processing is based on consent; the withdrawal of consent does not affect the legality of the processing performed before the withdrawal). The data subject is also entitled to lodge a complaint with the President of the Office for Personal Data Protection (ul. Stawki 2, 00-193 Warsaw) against the Controller if he/she considers that the processing of personal data violates the provisions of the law.   10. The provision of your personal data is voluntary but necessary for technical reasons in order to stay on the premises of the Administrator's treatment facility.
