Venous surgery The Phlebology Clinic / Services / Venous surgery Safenectomy Miniflebectomy Hybrid treatments Introduction Surgical treatment of veins has a long history and has undergone significant evolution over more than 100 years. 1 The beginnings of vein surgery date back to antiquity, but the real breakthrough came in the 20th century with the development of minimally invasive techniques. The first mentions of varicose vein treatment come from ancient Egypt and Greece, where bloodletting and herbal poultices were used. In the Middle Ages, the use of compression bandages and herbal extracts became widespread. In the 19th century, surgical techniques such as ligation and removal of varicose veins began to be introduced, but they carried a high risk of complications. The 20th century saw the development of anesthesia and antiseptic techniques, which effectively contributed to the popularization of surgical procedures. Over time, the introduced sclerotherapy became a less invasive alternative to surgical operations. The recent years (21st century) represent a true breakthrough in the treatment of venous disease thanks to the enormous progress in vein diagnostics and imaging, as well as the development of minimally invasive techniques such as percutaneous laser ablation, vein gluing, and ultrasound-guided foam sclerotherapy. The place of surgical vein treatment in contemporary phlebology Contemporary phlebology emphasizes a comprehensive and interdisciplinary approach to the treatment of venous disease. Surgical vein treatment still plays an important role, but it is no longer the only solution. The decision to use surgery is made individually, depending on the stage of the disease, symptoms, and patient preferences. Modern surgical methods aim towards a significant minimization of invasiveness. Currently, the most commonly performed procedure is microphlebectomy, which involves the removal of varicose veins through minimal skin incisions, without the need for sutures, and is performed under tumescent local anesthesia. Tumescent anesthesia is a special type of local anesthesia that reduces pain and discomfort during the procedure, as well as limiting bleeding and the risk of complications. Surgical vein treatment is typically one element of a comprehensive therapy for venous disease based on the philosophy of causal treatment, implemented by our phlebologist doctors several years ago. The modern approach to venous disease therapy is based on a holistic diagnosis of the venous system, analysis of risk factors, and then on an individualized treatment process. The entire therapeutic process consists of several stages. Despite the dynamic development of minimally invasive endovenous methods, surgical vein treatment in the contemporary sense still has significant importance, especially in advanced stages of lower limb venous disease. Did you know? Surgical treatment of venous disease has undergone a true technological revolution in recent years – from classic surgeries to minimally invasive procedures performed on an outpatient basis.
